Job Description

We are seeking an enthusiastic and motivated

Registered Nurse (Band 7)

to join our forward‑thinking

Pre‑assessment and Population Health Management Team

at West Middlesex Hospital.

This is a unique and exciting opportunity to establish and develop the Trust’s first

Population Health Management Clinic

, while also playing a key leadership role within the

Pre‑assessment (POA) service

. The post holder will work collaboratively with hospital and community teams to deliver a high‑quality, patient‑centred service aimed at improving both

individual surgical readiness and wider population health outcomes

.

The role combines

clinical expertise, leadership, and service development

, with responsibility for assessing patients prior to surgery, identifying and managing risks, and ensuring patients are optimised for safe procedures. This includes reviewing medical history, performing clinical assessments, arranging appropriate investigations, and providing education to support informed decision‑making.

In parallel, the post holder will lead on

prevention, early intervention, and lifestyle modification

, supporting patients to reduce risk factors such as smoking, poor diet, and inactivity, thereby improving long‑term health outcomes and reducing health inequalities.

  • Undertake comprehensive pre‑operative assessments in line with agreed protocols
  • Identify and manage clinical risks, ensuring patients are optimised for surgery
  • Request, interpret, and act on investigations (e.g. blood tests, ECGs)
  • Provide patient education on procedures, recovery, and lifestyle improvements
  • Deliver and develop population health interventions, including behaviour change support
  • Work collaboratively with community public health teams to support continuity of care

Contribute to the development of the nurse‑led population health clinic
