JOB SPECIFICATIONS
QUALIFICATION
Basic Qualification / Education / Vocational Training
- Bachelor’s Degree in Nursing from an accredited institute/college/university.
- Valid DHA License as Registered Nurse.
Advanced Qualification / Certification / Specialist Training
- BLS Certification mandatory.
- Home Healthcare experience preferred.
Experience
Minimum Experience
- Minimum two (2) years of experience as Registered Nurse in clinic, hospital, or home healthcare setting.
Specific / Relevant Experience
- Experience in patient coordination, scheduling, and home healthcare operations is preferred.
- Experience handling multidisciplinary healthcare teams and patient communication.

OBJECTIVE
- Coordinate and oversee home healthcare services and patient scheduling.
- Ensure safe, efficient, and high-quality delivery of home healthcare services.
- Act as liaison between patients, families, physicians, nurses, and operations teams.
- Maintain compliance with DHA standards, documentation, and patient safety protocols.
- Support operational efficiency and continuity of care for home healthcare patients.
Job Specific Responsibilities
GENERAL RESPONSIBILITIES
- Coordinate home healthcare appointments, scheduling, and staff allocation.
- Assess patient requirements and coordinate suitable healthcare professionals for home visits.
- Follow up with patients and families regarding appointments, treatment plans, and ongoing care.
- Ensure all required patient documentation, consents, and records are completed accurately.
- Monitor daily home care operations and ensure smooth coordination of services.
- Support nurses and physicians in planning and execution of home healthcare services.
- Maintain communication with multidisciplinary healthcare teams regarding patient updates and requirements.
- Ensure compliance with DHA regulations, infection control, and patient safety standards.
- Coordinate medical supplies, medications, and equipment required for home visits.
- Handle patient complaints, concerns, and service recovery professionally.
- Monitor staff attendance, scheduling conflicts, and operational coverage for home care services.
- Prepare operational and activity reports as requested by management.
- Participate in quality improvement and patient safety initiatives.
- Attend mandatory trainings, meetings, and competency assessments.
- Assist with onboarding and orientation of home healthcare staff when required.
- Perform nursing duties during home visits if operationally required.
Clinical Responsibilities
- Conduct nursing assessments during home visits when required.
- Monitor patient condition and escalate concerns to physicians appropriately.
- Administer medications and treatments within nursing scope and DHA regulations.
- Educate patients and families regarding treatment plans, medications, and home care instructions.
- Ensure infection control and patient safety standards are maintained during home visits.
- Maintain accurate nursing and operational documentation.
Occupational Health \& Safety (ohs) Responsibilities
- Comply with clinic and home healthcare OHS policies and procedures.
- Ensure safe handling and transportation of medical equipment and supplies.
- Report incidents, hazards, and unsafe practices immediately.
- Correctly utilize PPE and infection control measures during patient visits.
- Participate in emergency preparedness and safety trainings.
