**Clinical Team Manager, RN (CCM/Population Health) – Remote** Full\-Time \| Fully Remote \| $110K–$130K Based on Experience We’re hiring an experienced **Clinical Team Manager, RN,** to lead a remote nursing team supporting patients with chronic conditions in a structured care management model. This role is ONLY for RNs with prior **team leadership experience in care management (CCM, PCM, population health).** *If your background is primarily hospital\-based or focused on staffing/coverage, this will not be a fit* . **What You’ll Do** * Lead a team of nurses managing longitudinal patient panels (not episodic care) * Oversee quality across patient interactions, documentation, and follow\-up * Drive performance metrics, productivity, and accountability * Coach and develop nurses in a structured environment * Build and improve workflows, SOPs, and team processes * Support hiring, onboarding, and scaling of the clinical team **What We’re Looking For** * Registered Nurse (RN) * Prior **team leader, supervisor, or management experience** * Background in **CCM, PCM, or population health** * Experience working in **structured, metric\-driven environments** * Comfortable managing **remote teams** * Strong understanding of **panel\-based, longitudinal care** **Not a Fit If You:** * Are primarily hospital\-based (ER, ICU, inpatient leadership) * Have not directly managed nurses or owned team performance * Work mainly in utilization review, audits, or non\-patient\-facing roles * Prefer episodic care vs ongoing patient management **Details** Fully remote (U.S.\-based) Base salary: $110K–$130K **Apply** * If you’ve led nurses in a care management setting and are comfortable owning team performance and outcomes, we’d love to connect.
