Chronic Care Management (CCM)

Role Description

This is a full-time, on-site role for a Chronic Care Management professional based in Lahore. The role involves providing excellent Patient Support service, effective communication with patients and healthcare providers, supporting chronic care coordination, and understanding the principles of medicine and critical care. Day-to-day tasks will include patient engagement, tracking care plans, and ensuring high-quality services for continuous patient management.

• Excellent communication skills to interact with patients, healthcare providers, and team members
• Basic understanding of medicine and critical care management practices
• Ability to work collaboratively on patient-centered care initiatives
• Relevant experience in healthcare or medical billing industries is a plus
• Bachelor’s degree in Pharm-D
