Job Description
- 10AM-10PM
- Discusses facility core measures during handoff communication.
- Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.
- Collaborates with interdisciplinary team members to solve admission and discharge problems.
- Collects comprehensive patient medical history. Establishes and maintains positive relationships with patients and their families.
- Provides family members with information about patient condition and plan of care.
- Promotes a positive patient experience by demonstrating caring behaviors, maintaining clear communication, and collaborating with the care team to provide safe, timely, and effective whole person care.
- Follows hospital policies and procedures, making ethical decisions based on consent and confidentiality.
- Collaborates with nursing staff and attending physicians in establishing patient admission and discharge processes.
- Ensures compliance with healthcare admission policies and regulations. Maintains a safe and clean environment for patients.
- Manages all aspects of patient admission and discharge processes.
- Prepares, completes, and registers patient admissions.
- Other duties as assigned.

Education
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
