You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management. * Provide patient\-focused, safe, holistic nursing care and work collaboratively with other members of the health care team * Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes * Continually assess, anticipate, identify and respond to new and changing needs of our patients * Travel within your region between patients’ homes Qualifications * Minimum two years nursing experience (Community Nursing experience is ideal) * Registered and in good standing with the College of Nurses of Ontario (CNO) * Ability to work independently in the community * Proficiency in English, both verbal and written required (additional languages an asset) * Valid Ontario driver's license and reliable vehicle * Experience with infusion therapy, mental health, palliative care, and wound care * Excellent patient assessment and teaching skills \#TO\_nurse Additional Information This position is a current vacancy
