Job Description
**Registered Nurse St Michael’s Family Health Team** The RN role in the Department of Family and Community Medicine works within a changing environment and in inter\-professional teams to advocate for and promote wellness while ensuring the delivery of safe, effective and ethical nursing care. As a member of the interprofessional primary care team, the RN provides comprehensive primary care services to individuals and their families across the lifespan and supports health promotion and disease prevention initiatives in optimizing health for our patients and families. The RN shares their knowledge and expertise with others and contributes to the advancement of nursing practice excellence. The RN continually enhances his/her practice and competencies through ongoing learning, education, experience, and reflective practice. The primary location of this position is across all 5 Family Health Team sites as needed. Please see the current location of our clinics below: * 61 Queen St. East * 80 Bond Street * 140 The Esplanade * 95 Homewood Ave * 73 Regent Park Blvd This job requires coverage for regular day hours (9am\-5pm) and after hour clinics (12pm\-8pm), including evening and weekend/stat holiday shifts. **Responsibilities** The primary role of the RN is to provide holistic, evidence\-based, culturally appropriate, patient\-centred quality care in accordance with the College of Nurses of Ontario standards and Unity Health Toronto’s mission, vision, values and policies. The RN optimizes the patient’s experience and care outcomes by: * Being accountable for ones decisions and professional practice; * Establishing a therapeutic relationship with the patient; * Collaborating with other members of the patient’s care team; and, * Coordinating and communicating patient care activities. * Health teaching and assessment (history and physical assessment) of a variety of patients from all ages across the life span; newborns, infants, toddlers, school aged, adolescents, young adults, adults and elderly * Health teaching and assessment (history and physical assessment) of a variety of patient conditions; including, prenatal and postnatal families, acute/episodic care and chronic disease management including (but not limited to): hypertension, elevated cholesterol, Asthma/COPD, cardiovascular disease, smoking cessation, diabetes and mental health. * Triaging and independent urgent care assessments for patients in office visits and by telephone to collect information about patient medical care need and urgency. * Coordinating and case managing complex patient care with the home visit team and other community services. * Communicating with patients regarding lab results and providing health teaching. * Independent assessment (history and physical assessment) for annual adult wellness visits (physical exams) and well\-baby visits. * Providing infant, child and adult immunizations, injections, wound care, ear syringing, and EKG's. * Providing health teaching and health promotion to patients, families and groups to help them achieve optimal health status. * Participate in cancer care screening initiatives. * Contributing to the quality improvement initiatives and the academic and research functions of the department. **Patient Care** The RN has 4 key areas of responsibility: * Acquires information to assess patient care and learning needs * Develops a plan of care and integrates nursing activities with other inter\-professional activities * Engages in critical inquiry and reflection to select evidence\-based interventions * Organizes nursing care and assigns or changes nursing care activities where appropriate * Evaluates the effectiveness of the plan of care and adjusts the plan as needed * Anticipates, recognizes and addresses rapidly changing situations * Provides health education to patients and their families using effective teaching strategies * Documents observations, assessments and care provision * Contributes to discharge and transition planning * Refers patient to other agencies or services as needed **Collaboration \& Coordination** * Serves as the primary resource for organizing and coordinating patient care activities * Establishes and maintains respectful, collaborative, therapeutic and inter\-professional relationships * Coordinates and communicates patient care activities for a safe and seamless transition and appropriate transfer of accountability * Participates in hospital committees and other groups **Leadership** * Aligns behavior with Unity Health Toronto’s vision, mission and values and Code of Conduct * Supports the advancement of professional practice * Utilizes effective judgment in assigning patient care activities * Engages in self\-reflection, performance review and ongoing learning * Participates in mentoring, teaching and /or coaching students, co\-workers and other regulated and unregulated health professionals * Embraces change that leads to improved nursing practice and better patient outcomes * Advocates for a safe and healthy work environment * Research * Participates in or supports research projects * Supports and participates in evidence based practice implementation and quality improvement initiatives **Qualifications** * Current Registration with College of Nurses of Ontario (General Class) * Primary Care, Community, or Emergency nursing experience is required * BLS Certification is required * Ability to work collaboratively with other professions / disciplines in a team environment * Excellent communication skills * Proven planning, time management, organization and coordination skills * Skills in triaging patients in person and by telephone * Strong care coordination skills with services in the hospital and community * Demonstrated patient/family teaching experience. * Demonstrated leadership skills within an interdisciplinary team environment * Must have excellent organizational skills and demonstrated ability to successfully work in a fast\-paced multidisciplinary out\-patient setting * Relevant knowledge, skills and competence including immunizations, injections, and wound care Unity Health Toronto is committed to creating an accessible and inclusive organization.
