Medical Coding QA Associate (Home Health QA)

**Job Title:** Offshore QA Associate (Home Health QA) **About the Role:** Our client is seeking an experienced and dependable QA Associate (Home Health QA) to join their growing home health team. Ideal candidates will perform QA reviews of home health documentation (OASIS, SOC, Recert, Discharge) to ensure clinical accuracy, completeness, and compliance. Support ICD\-10 coding accuracy, medication reconciliation, and plan\-of\-care readiness, allowing field RNs to focus on patient care. This individual will be responsible for reviewing and ensuring quality and compliance across clinical documentation, including but not limited to: * OASIS submissions (accuracy, timeliness, and compliance) * Skilled Nursing and Therapy visit notes * CMS\-485 (Plan of Care) reviews * Recertifications and discharge summaries * Ensuring documentation aligns with Medicare guidelines and supports billing/ADR requirements **Key Responsibilities:** * Documentation QA: Review documentation (OASIS/SOC/Recert) for accuracy, consistency, and clear support of skilled need/patient status. Provide specific correction guidance to clinicians. * ICD\-10 Coding: Validate and ensure primary/secondary ICD\-10 codes align with documentation and home health rules. * Medication Review: Perform QA on medication reconciliation; identify and escalate safety concerns (duplicates, contraindications, adherence). * Order Support: Ensure documentation supports Physician Orders/POC readiness. * Compliance: Track errors, maintain QA logs, and support continuous improvement efforts. **Qualifications** : * Medical Doctor or Registered Nurse (RN) qualification (Pakistan\-based acceptable). * Strong understanding of physiology, pharmacology, and Home Health workflow. * ICD\-10 proficiency (home health preferred). * Excellent English writing/reading skills. **Preferred Qualifications:** Prior experience in OASIS review/QA, home health auditing, clinical coding support, or US healthcare documentation standards. **Skills:** High attention to detail, strong clinical judgment, clear/professional feedback, meeting turnaround targets, and maintaining strict confidentiality (HIPAA\-style).
